چکیده

Pointreau et al. (1) clearly showed that docetaxel, cisplatin, and 5-fluorouracil (TPF) is better than PF for larynx preservation in their important phase III study. Unfortunately, we are only given results for larynx preservation as an indicator of a successful laryngeal outcome. It is important to address the issue of laryngectomy-free survival, too, because that endpoint has broader implications. Laryngectomy-free survival accounts for those patients who survived with an intact and functional larynx instead of only focusing on those with a preserved larynx. Two endpoints that are considered by many to be important in larynx preservation trials are not presented in the study. These are prolonged trache-otomy and permanent percutaneous endo-scopic gastrostomy dependency (2 , 3). Interpretation and implication of the results of the trial will be strengthened by these data. It is also not clear why a total larynge-ctomy was chosen as the operation of choice for these patients instead of a partial laryngectomy in which a portion of the larynx was preserved. Finally, five patients

Randomized trial of induction chemotherapy with cisplatin and 5-fluorouracil with or without docetaxel for larynx preservation.

BACKGROUND Chemotherapy with cisplatin (P) and 5-fluorouracil (F) followed by radiotherapy in patients who respond to chemotherapy is an alternative to total laryngectomy for patients with locally advanced larynx and hypopharynx cancer. Data suggest that docetaxel (T) may add to the efficacy of PF. The objective of this trial was to determine whether adding T to PF could increase the larynx pre...

Phase II organ-preservation trial: Concurrent cisplatin and radiotherapy for advanced laryngeal cancer after response to docetaxel, cisplatin, and 5-fluorouracil-based induction chemotherapy.

BACKGROUND The optimal treatment for locally advanced laryngeal cancer remains controversial. The purpose of this trial was to determine if the response to induction chemotherapy could select patients for organ preservation protocols, and improve larynx-preservation rates without compromising overall survival (OS). METHODS The cohort comprised 12 patients with T3 disease and 14 with T4. Induc...

Concurrent chemoradiotherapy for organ function preservation in advanced patients with hypopharyngeal and laryngeal cancer.

Preservation of the larynx is the most critical factor influencing quality of life in the treatment of head and neck cancer. This clinical study focuses on laryngeal function-preserving chemoradiotherapy for locally advanced hypopharyngeal and laryngeal cancer. Thirty-two resectable cases with histologically proven squamous cell carcinoma undergoing function-preserving therapy were examined. In...

Cisplatin and fluorouracil alone or with docetaxel in head and neck cancer.

BACKGROUND A randomized phase 3 trial of the treatment of squamous-cell carcinoma of the head and neck compared induction chemotherapy with docetaxel plus cisplatin and fluorouracil (TPF) with cisplatin and fluorouracil (PF), followed by chemoradiotherapy. METHODS We randomly assigned 501 patients (all of whom had stage III or IV disease with no distant metastases and tumors considered to be ...

Induction chemotherapy with TPF (Docetaxel, Carboplatin and Fluorouracil) in the treatment of locally advanced squamous cell carcinoma of the head and neck.

Squamous cell carcinoma of the head and neck (SCCHN) presents at a locally advanced (LA) stage in many patients. Chemotherapy, which is one fundamental therapy mode for local disease control of inoperable disease or if organ preservation is desired, has become an important factor of first line treatment regimens either during or prior to radiotherapy (RT). Patients with locoregionally advanced ...
